Metabolic Health Markers Your Primary Care Doctor May Not Be Checking, and Why They Matter for Weight Management

Most annual physicals include a fasting glucose, a basic lipid panel, and maybe a hemoglobin A1c if you're over 45 or carrying extra weight. That's it. If those numbers land in the "normal" range, you get a pat on the back and a reminder to exercise more. Then you leave wondering why the scale hasn't budged in two years despite doing everything you've been told.

The problem isn't you. The problem is that standard labs were designed to catch overt disease, not to explain why your metabolism is slowing down or why fat keeps accumulating around your midsection. In our practice, we routinely order a second tier of markers that tell a much more useful story. Here are the ones I find most valuable, and what they can reveal.

Fasting Insulin (and the HOMA-IR Calculation)

Glucose can look perfectly normal for years while insulin quietly climbs. Your pancreas compensates, pumping out more and more insulin to keep blood sugar in range. By the time fasting glucose finally rises, you've often been insulin resistant for a decade.

Fasting insulin is a simple blood draw. Pair it with fasting glucose and you can calculate HOMA-IR, a rough index of insulin resistance. I like to see fasting insulin under 7 µIU/mL. When it's sitting at 15 or 20 with "normal" glucose, that's the person who tells me they gain weight looking at a bagel. They're not exaggerating. High circulating insulin is a fat-storage signal, and it also blunts the enzymes that release stored fat for energy.

ApoB and LDL Particle Number

Your standard lipid panel measures the cholesterol carried inside LDL particles. What it doesn't tell you is how many particles are floating around. Two people can have the same LDL-C of 110 mg/dL, but one has fewer, larger particles and the other has a swarm of small, dense ones. The second person carries a meaningfully higher cardiovascular risk.

ApoB counts the atherogenic particles directly. It correlates tightly with metabolic dysfunction because insulin resistance drives production of small, dense LDL and triglyceride-rich remnants. When I see a patient with a normal LDL-C but an ApoB north of 100 mg/dL and triglycerides over 130, I know we're dealing with a metabolic problem, not just a cholesterol problem. Addressing the weight and insulin issue often improves both.

Triglyceride-to-HDL Ratio

This one is free. It's already on your standard lipid panel. Divide your triglycerides by your HDL. A ratio under 1.5 suggests good metabolic health. Over 3.0 strongly suggests insulin resistance, even if every other number looks tidy. I've watched this ratio predict who will respond quickly to nutritional changes and who has deeper metabolic work to do.

hs-CRP

High-sensitivity C-reactive protein measures low-grade systemic inflammation. Visceral fat, the kind packed around your liver and intestines, is metabolically active and secretes inflammatory signals. Chronically elevated hs-CRP tells me a patient's fat tissue is behaving badly, and that inflammation itself is contributing to insulin resistance and difficulty losing weight.

An ideal hs-CRP sits below 1.0 mg/L. Values over 3.0 warrant attention, assuming you don't have an acute infection skewing the result.

Uric Acid

Most people associate uric acid with gout. It's also a sensitive marker of fructose exposure and metabolic stress. Elevated uric acid is associated with hypertension, fatty liver, and insulin resistance. When I see a level over 6 mg/dL in a patient struggling with weight, we start looking at liquid calories, ultra-processed foods, and alcohol intake with fresh eyes.

GGT and ALT

Liver enzymes get glanced at and dismissed if they're within lab range. But the "normal" upper limit for ALT was set decades ago using a population that included plenty of people with undiagnosed fatty liver. A more useful ceiling is around 30 U/L for men and 25 U/L for women. GGT rising in tandem often points to metabolic-associated fatty liver disease, which affects roughly one in three American adults and is a major driver of stubborn weight gain and insulin resistance.

You can have completely normal AST and total bilirubin while your liver is quietly filling with fat. Catching this early matters, because fatty liver is one of the most reversible metabolic problems we deal with.

Thyroid Beyond TSH

TSH alone is a lazy thyroid screen. A patient can have a "normal" TSH of 3.8 and feel exhausted, cold, and unable to lose weight. I want to see free T4, free T3, and often reverse T3 and thyroid antibodies. Conversion from T4 to the active T3 can be impaired by stress, chronic dieting, inflammation, and nutrient deficiencies. If your free T3 is bumping along the bottom of the range, your resting metabolic rate is likely lower than it should be, and no amount of caloric restriction will fix that comfortably.

Sex Hormones and SHBG

Testosterone in men and estradiol, progesterone, and testosterone in women shift body composition in predictable ways when they drift out of range. Low testosterone in men correlates with increased visceral fat and reduced insulin sensitivity. In perimenopausal women, falling progesterone and fluctuating estradiol change where fat is stored and how the body responds to insulin.

SHBG (sex hormone-binding globulin) is a marker I watch closely. Low SHBG is a red flag for insulin resistance, often before glucose or A1c budge. Hormone optimization, when clinically appropriate, can be part of a broader Weight Management strategy, but only after the metabolic picture is understood.

Vitamin D, Ferritin, and Magnesium

These aren't glamorous, but deficiencies quietly undermine metabolic function. Vitamin D acts more like a hormone than a vitamin and influences insulin secretion. Low ferritin blunts energy, exercise tolerance, and thyroid conversion. Magnesium is a cofactor in hundreds of enzymatic reactions, including glucose handling, and most Americans don't get enough from diet alone.

Why This Matters if You're Trying to Lose Weight

When someone comes to us frustrated after months of dieting with nothing to show, the answer is almost never "try harder." It's usually that one or more of these markers is out of range, and until we address the underlying biology, calorie math won't cooperate. High fasting insulin, sluggish free T3, an inflamed liver, low testosterone: each of these makes fat loss dramatically harder, and each is treatable once identified.

A thoughtful Weight Management program starts with knowing which levers actually need pulling for your body. That requires better data than a five-marker screening panel can provide.

What to Do With This Information

You can ask your primary care doctor to add several of these labs. Some will oblige. Others will decline because insurance won't cover markers outside standard screening guidelines.
