
Hormone Optimization for Men Over 40: What Comprehensive Lab Testing Can Reveal
Most men who come to see me in their 40s and 50s don't lead with the word "hormones." They lead with fatigue that coffee no longer touches, a midsection that won't respond to the same gym routine that worked at 35, sleep that feels shallow, and a quiet erosion of drive at work and at home. When we finally run comprehensive labs, the picture almost always tells a more interesting story than a single testosterone number could.
The standard physical you get through most primary care offices was designed to catch disease, not to characterize how your endocrine system is actually performing. That's a real gap, because by the time a conventional panel flags "low T," you've often spent years feeling off without any objective explanation. Comprehensive testing changes the conversation.
Why a Single Testosterone Number Misleads Men
Total testosterone is the number most men know, and it's the one most doctors order. It's also, in isolation, close to useless. Two men can walk into my office with identical total testosterone levels of 550 ng/dL and have completely different clinical realities. One feels sharp, sleeps well, and has a libido he's happy with. The other is exhausted, anxious, and hasn't had a morning erection in eight months.
The difference usually lives in the numbers most panels never measure: free testosterone, sex hormone binding globulin (SHBG), estradiol, dihydrotestosterone (DHT), and the upstream signals from the pituitary. Testosterone doesn't work alone. It works inside a system, and that system shifts meaningfully after 40.
What a Comprehensive Male Hormone Panel Actually Includes
When we work up a man for hormone optimization, we're looking at roughly three dozen markers, drawn in the morning when testosterone peaks. The core panel includes:
- Total and free testosterone. Free testosterone is the fraction actually available to your tissues. It can be low even when total looks normal.
- SHBG. This binding protein rises with age, with alcohol intake, and with thyroid shifts. High SHBG sequesters testosterone and leaves you symptomatic despite decent total levels.
- Estradiol (sensitive assay). Men need estrogen. Too little causes joint pain, poor sleep, and low libido. Too much drives moodiness, water retention, and gynecomastia. The sensitive assay matters because standard estradiol testing was designed for women and reads male levels inaccurately.
- DHT. The more potent androgen derived from testosterone, relevant to libido, hair, and prostate.
- LH and FSH. The pituitary signals that tell your testes to produce. Their pattern tells us whether a problem is primary (testicular) or secondary (pituitary or hypothalamic).
- Prolactin. Elevated prolactin can suppress testosterone and occasionally signals a pituitary adenoma worth investigating.
- Thyroid panel with TSH, free T3, free T4, and reverse T3. Thyroid dysfunction mimics low testosterone symptoms almost perfectly.
- DHEA-S and cortisol. Adrenal output shapes energy, stress tolerance, and how well testosterone actually performs at the tissue level.
- Fasting insulin, glucose, and HbA1c. Insulin resistance suppresses SHBG, raises estradiol through increased aromatase activity in fat tissue, and drops free testosterone. You can't fix hormones without addressing this.
- Comprehensive metabolic panel, lipids, CBC, ferritin, vitamin D, and PSA. The supporting cast that determines whether optimization is safe and what else needs attention.
We add hs-CRP, homocysteine, and sometimes apolipoprotein B when cardiovascular risk needs a closer look, which in men over 40 is almost always.
The Patterns We See in Practice
A few clinical pictures repeat often enough that they're worth naming.
The High-SHBG Executive
Total testosterone reads 600, which every prior doctor called "fine." SHBG is 75 nmol/L (upper end of normal). Free testosterone is at the bottom of the range. He's fatigued, foggy, and losing muscle despite training. The number that mattered was never on his old labs.
The Metabolic Cascade
Fasting insulin is 18, HbA1c is 5.9, SHBG is 18, estradiol is 42, and total testosterone is 380. This man doesn't primarily have a testosterone problem. He has insulin resistance driving aromatization of testosterone into estradiol in visceral fat. Optimizing his hormones without addressing metabolism would be treating downstream of the actual issue.
Secondary Hypogonadism in the 40-Something
Total testosterone of 250, LH of 2.1, FSH of 2.4. The testes aren't the problem. The signal from above is muted, often related to sleep debt, chronic stress, subclinical hypothyroidism, or elevated prolactin. The workup matters because the treatment path differs meaningfully depending on the cause.
Thyroid Masquerading as Low T
I've seen men on testosterone protocols for two years who still felt terrible because nobody checked free T3 and reverse T3. Correct the thyroid and the testosterone works the way it was supposed to.
What Labs Can't Tell You
Numbers frame the conversation, they don't finish it. A man with textbook-normal labs who feels awful deserves more investigation, not dismissal. A man with borderline-low testosterone who feels great may not need intervention at all. In our practice, we treat the person in the chair using the labs as one input among several, alongside symptoms, physical exam, body composition, sleep quality, and what he actually wants his next decade to look like.
The other honest limitation: reference ranges reflect the population being tested, which increasingly means tired, overweight, poorly-sleeping men. "Normal" in that context is not the same as "optimal." A testosterone level typical of a sedentary 55-year-old is not what you want if you plan to be strong and sharp at 70.
How We Use the Data
Once we have a complete picture, the plan is specific to what the labs reveal. Some men need targeted lifestyle work first, particularly around sleep, resistance training, alcohol, and metabolic health, because their numbers will move meaningfully with those changes alone. Others are candidates for hormone optimization protocols involving testosterone, along with adjuncts to manage estradiol conversion or preserve fertility when relevant. A subset needs thyroid support, adrenal work, or treatment of an underlying pituitary issue before testosterone is even on the table.
We recheck labs on a defined schedule, usually at six weeks, twelve weeks, and then quarterly, adjusting doses based on both numbers and how you feel. Optimization is not a one-time prescription. It's an ongoing calibration.
When to Consider a Full Workup
If you're a man over 40 experiencing persistent fatigue, declining libido, difficulty maintaining muscle, weight gain around the middle that won't respond to your usual efforts, mood changes, poor sleep, or brain fog, a comprehensive panel will almost always clarify what's happening. Even if you feel reasonably well, a baseline in your 40s is one of the more valuable data points you can have for the next twenty years.
