
GLP-1 Medications and Hormone Optimization: How Balancing Both May Support Sustainable Weight Management
The patient in front of me had lost 34 pounds on semaglutide over eight months. She was thrilled, then frustrated. The scale had stalled for six weeks. Her energy was lower than before she started. Sleep felt shallow. Libido had quietly disappeared. Her labs told a story her prescriber had missed: testosterone in the bottom decile for her age, a free T3 hovering near the floor of the reference range, and a morning cortisol pattern that suggested her body was fighting the caloric deficit harder than her willpower ever could.
This is the conversation I find myself having weekly. GLP-1 medications are extraordinary tools. They also don't work in isolation, because your metabolism doesn't work in isolation. If we ignore the endocrine system while asking it to do something demanding, we get results that look good on the scale for a while and then quietly unravel.
What GLP-1 Medications Actually Do
Semaglutide and tirzepatide mimic gut hormones (GLP-1, and in tirzepatide's case, GIP as well) that your intestines release after a meal. They slow gastric emptying, dampen appetite signaling in the hypothalamus, and improve insulin sensitivity. Tirzepatide adds a second incretin pathway, which is part of why the average weight reduction in trials was higher than semaglutide.
The mechanism is elegant. The problem is that appetite suppression alone doesn't guarantee that the weight you lose is the weight you wanted to lose. In our practice, we often see patients who dropped 15% of body weight without any resistance training or protein target, and roughly a third of that loss was lean mass. That matters at 42, and it matters even more at 55.
Where Hormones Enter the Picture
Your thyroid, adrenal, and sex hormones govern the environment in which weight change happens. Push a body into a sustained caloric deficit without checking on those systems, and predictable things occur.
Thyroid
The thyroid down-regulates in response to lower energy intake. T4 to T3 conversion slows. Reverse T3 can climb. Patients describe cold hands, thinning hair at the temples, constipation, and a fatigue that coffee doesn't touch. A TSH within the "normal" range tells you almost nothing here. We want to see free T3, free T4, reverse T3, and antibodies, and we want to look at the pattern, not just the flags.
Sex Hormones
In men, adipose tissue produces aromatase, which converts testosterone to estradiol. Significant weight change shifts that balance, sometimes helpfully, sometimes not. A man who starts with obesity-related low testosterone may see modest improvement as he loses fat. He may also plateau in a place where free testosterone is still insufficient for the muscle preservation he needs on a GLP-1.
In women, particularly in perimenopause and the years after, estradiol and progesterone shifts influence where fat is stored, how insulin behaves, and how well sleep restores the nervous system. A woman who starts a GLP-1 during perimenopause without addressing hormonal changes often loses weight initially and then finds the last 10 to 15 pounds immovable, while sleep and mood deteriorate.
Cortisol and Insulin
Chronic stress and poor sleep raise cortisol, which encourages visceral fat storage and blunts the weight change you'd expect from the medication. Insulin resistance, even when fasting glucose looks fine, changes how aggressively the body defends stored fat. Fasting insulin and HOMA-IR are cheap tests that almost nobody runs, and they change the plan meaningfully.
Why Combining the Two Requires Real Clinical Attention
The appeal of pairing GLP-1 therapy with hormone optimization isn't that one supercharges the other. It's that they address different problems that both need addressing if you want the result to last past your last injection.
Consider the 48-year-old woman with 40 pounds to lose, poor sleep, a stalled career of gym attendance, and estradiol dropping through perimenopause. A GLP-1 alone will reduce her appetite. It won't restore the sleep that lets her recover from workouts. It won't rebuild the lean mass she's been slowly losing since 40. Hormone optimization, done conservatively and monitored closely, can address the underlying environment. The GLP-1 addresses the appetite dysregulation. Together, with resistance training and adequate protein (I generally target 1.6 to 2.2 grams per kilogram of goal body weight), the picture changes.
Consider the 54-year-old man carrying visceral fat, snoring, with a total testosterone of 320 and a free T of 5.8. Putting him on tirzepatide will drop his weight. If we ignore the testosterone, he'll lose muscle he can't afford to lose, his sleep apnea won't necessarily improve as quickly as his waistline suggests it should, and his energy for training will be poor. Addressing both, in sequence and with appropriate monitoring, is a different clinical proposition entirely.
What Careful Practice Looks Like
A few principles guide how we approach this in our practice:
- Baseline labs before anything starts. Comprehensive thyroid panel, sex hormones with SHBG and free fractions, fasting insulin, HbA1c, lipid panel with ApoB, inflammatory markers, and a metabolic panel. Add DEXA when body composition matters, which is usually.
- Start one intervention at a time when possible, so we can see what's doing what. GLP-1 titration is slow for a reason.
- Protein and resistance training are non-negotiable. If a patient tells me they don't have time to lift, we have a different conversation before we prescribe anything.
- Recheck labs at 90 days, then quarterly. Hormones shift as weight shifts. Doses that were correct at the start rarely stay correct.
- Have an exit plan. GLP-1 medications are not always meant to be lifelong. What does maintenance look like at a lower dose, or without the medication? We plan for that from the first visit.
The Side Effects Nobody Warns You About
Nausea and constipation get the airtime. The ones I see cause the most trouble are subtler: muscle loss that shows up on DEXA before it shows up in the mirror, a flattening of mood that patients often attribute to life stress, and hair shedding around month four that's usually a combination of rapid weight change and untreated thyroid patterns. None of these are reasons to avoid the medication. They are reasons to have someone paying attention.
Who This Combination Isn't Right For
Not everyone with weight to lose needs hormone optimization. Not everyone with a hormone imbalance needs a GLP-1. Someone with a personal or family history of medullary thyroid carcinoma shouldn't take these medications. Someone whose testosterone is genuinely normal doesn't need it manipulated. The point of individualized care is that the answer to "should I be on both?" is almost always "let's look at your data and decide."
I'd rather have a patient on nothing for three months while we correct sleep, protein intake, and training than start two interventions that make lab interpretation impossible for the next year.
The Longer View
The patients who do best with GLP-1 medications in our practice are the ones who treated the prescription as one piece of a broader plan. They lifted heavy things. They ate protein at every meal. They had their hormones evaluated properly, and treated when clinically appropriate. They didn't try to lose 40 pounds in 12 weeks. They understood that the goal wasn't just a smaller body, but a metabolically healthier one that could hold that weight for the next 20 years.
