
Regenerative Medicine for Joint Pain: What High-Performing Adults Should Know Before Exploring PRP or Prolotherapy
If you're in your forties or fifties and still training hard, running your business, chasing your kids around a tennis court, joint pain is not a minor annoyance. It's the thing that quietly reorganizes your week. You skip the Saturday ride. You start icing your shoulder after every board meeting. You wonder whether the cortisone shot your orthopedist mentioned is really the right next step, or just the easiest one.
In our practice, this is the conversation I have most often with high-performing patients: what to do about a joint that isn't broken enough for surgery but isn't behaving either. Regenerative options like platelet-rich plasma (PRP) and prolotherapy sit squarely in that middle ground. They're worth understanding on their own terms, not as miracle stories and not as snake oil.
What PRP and Prolotherapy Actually Are
PRP starts with your own blood. We draw a standard tube, spin it in a centrifuge to concentrate the platelets several times above baseline, then inject that concentrate directly into the injured tissue under ultrasound guidance. Platelets carry growth factors (PDGF, TGF-beta, VEGF, and others) that participate in the normal repair cascade. The premise is straightforward: deliver a dense population of these signaling molecules to a tendon or joint that hasn't healed on its own.
Prolotherapy is older and mechanically simpler. A dextrose solution, usually 12.5% to 25%, is injected into ligamentous attachments and joint capsules. The hyperosmolar solution creates a small, controlled irritation that recruits the body's inflammatory repair response to tissues that often have poor blood supply and tend to heal slowly on their own.
Both are office procedures. Both use your own biology or a simple sugar solution. Neither involves stem cells, and if a clinic is marketing "stem cell" injections to you for joint pain, ask hard questions about what's actually in the syringe.
Where These Protocols Fit Best
Regenerative injections work best for specific problems, not as a general-purpose fix for anything that aches. The applications with the most consistent clinical experience include:
- Mild to moderate knee osteoarthritis, particularly in patients who want to postpone or avoid arthroscopy
- Chronic lateral epicondylitis (tennis elbow) that hasn't responded to eccentric loading and rest
- Rotator cuff tendinopathy and partial-thickness tears
- Hip and gluteal tendinopathies, including greater trochanteric pain
- Plantar fasciitis that's persisted past six months
- Ligamentous laxity contributing to chronic low back or SI joint pain (a more common prolotherapy target)
Where these protocols are less useful: bone-on-bone end-stage arthritis, complete tendon ruptures, mechanical derangements that need surgical repair, and pain that's really coming from the spine rather than the joint you think it is. A careful diagnostic workup matters more than the injection itself. I've seen patients spend thousands on shoulder PRP when the real driver was a cervical radiculopathy.
What the Evidence Actually Shows
PRP for knee osteoarthritis has the strongest body of literature, with multiple randomized trials showing meaningful improvements in pain and function compared to hyaluronic acid or saline, particularly in earlier-stage disease. For lateral epicondylitis, PRP has outperformed corticosteroid injection at the one-year mark in several studies, largely because steroids give fast relief but appear to weaken the tendon over time.
Prolotherapy has a smaller evidence base but reasonable data for knee osteoarthritis, Osgood-Schlatter in adolescents, and chronic low back pain associated with ligamentous laxity.
What the studies don't tell you is how much variability exists in how these treatments are actually performed. PRP prepared on one system, spun at one speed, with one platelet concentration, injected without imaging, is not the same product as PRP prepared on a different system and delivered under ultrasound to a specific lesion. When you read that "PRP works" or "PRP doesn't work," the underlying protocols may have almost nothing in common. This is one reason I'm skeptical of blanket claims in either direction.
What to Expect if You Move Forward
A reasonable workup starts with a thorough history, physical exam, and usually recent imaging. For a knee, that means weight-bearing X-rays at minimum, often an MRI. For a shoulder or hip, ultrasound and MRI both have their place. If your clinician wants to inject before understanding what's actually damaged, slow down.
The procedure itself is typically 30 to 60 minutes. Expect soreness for several days afterward, sometimes a week. That post-injection inflammation is part of the mechanism, so we generally ask you to avoid NSAIDs during that window. Ice, acetaminophen, and relative rest are fine.
Most patients notice changes gradually over four to twelve weeks, not overnight. Many protocols involve a series of two or three injections spaced several weeks apart. Rehabilitation matters. An injection into a rotator cuff without addressing the scapular mechanics that overloaded it in the first place is a short-term intervention at best.
Questions Worth Asking Before You Book Anything
- Who is performing the injection, and are they using ultrasound guidance for anything deeper than a superficial tendon?
- What PRP preparation system is used, and what platelet concentration does it produce?
- How many sessions are being recommended, and what's the total cost before you start?
- What's the rehabilitation plan afterward, and who manages it?
- What happens if the first round doesn't produce meaningful improvement?
A clinician who welcomes these questions is a clinician worth working with. A clinic that pushes packages before it's examined you is not.
How This Fits Into a Broader Approach
The patients who do best with regenerative injections are usually the ones who don't view them as standalone interventions. If you're not sleeping well, your systemic inflammation is elevated. If your vitamin D is low, tissue repair is slower. If you're carrying an extra thirty pounds, your knee is going to keep telling you about it regardless of what we inject. If your training program has no deload weeks, no amount of PRP will keep pace with the damage.
