Testosterone and Men's Health: A Guide to Getting Your Levels Evaluated

Most men who walk into our practice asking about testosterone don't lead with the word. They describe something vaguer. Sleep that used to feel restorative but no longer does. A shorter fuse with their kids. Workouts that produce less than they used to, despite the same effort. A drop in morning erections. A creeping sense of flatness that they've been chalking up to age, stress, or the sheer weight of running a business and a household simultaneously.

Sometimes these symptoms point to testosterone. Sometimes they don't. The only way to know is to test properly, interpret carefully, and pair the numbers with an honest conversation about how you're actually living. This guide walks you through what a thorough evaluation looks like and what to expect if you decide to pursue one.

Why Testosterone Matters Beyond Libido

Testosterone is often reduced to a sex-drive hormone, which sells it short. Androgen receptors are distributed throughout the body: brain, bone, muscle, vascular endothelium, red blood cell precursors, even the sweat glands. When levels drop meaningfully below your personal set point, the downstream effects tend to show up in ways that don't obviously say "hormones."

In our practice, the most common complaints from men with low testosterone are fatigue that coffee doesn't fix, difficulty putting on or maintaining lean mass, visceral fat accumulation around the midsection, cognitive fog, low mood or irritability, and reduced morning erections. Libido changes are often present but rarely the loudest signal. Some men also notice their recovery from exercise takes noticeably longer, or that a poor night of sleep hits harder than it used to.

None of these symptoms are specific to testosterone. Thyroid disease, poor sleep, undiagnosed sleep apnea, depression, iron deficiency, medication side effects, and simple overtraining can all produce a similar picture. That's precisely why the workup matters.

When to Get Evaluated

There's no perfect age. We see men in their late thirties with clinically low levels and men in their sixties whose numbers still look like a graduate student's. Genetics, body composition, sleep quality, alcohol intake, chronic stress, and certain medications (opioids and long-term glucocorticoids in particular) all move the needle.

Reasonable triggers to get tested include:

  • A cluster of the symptoms above lasting more than a few weeks
  • Difficulty losing visceral fat despite reasonable diet and training
  • Unexplained changes in mood, motivation, or cognitive sharpness
  • Loss of morning erections or a meaningful shift in sexual function
  • A family history of hypogonadism or pituitary issues
  • Recovery from a period of significant illness, injury, or chronic stress

If you're simply curious about your baseline as part of a proactive health strategy, that's also legitimate. Knowing your numbers in your forties gives you a reference point for your fifties and sixties.

What a Proper Test Looks Like

This is where a lot of evaluations go sideways. A single total testosterone drawn at 2 p.m. after a stressful morning tells you almost nothing useful. Testosterone follows a diurnal rhythm, peaking in the early morning and drifting down through the day. Acute illness, poor sleep the night before, and even a hard workout can suppress readings.

A thorough baseline evaluation should include:

  • Total testosterone, drawn between 7 and 10 a.m., ideally on two separate occasions
  • Free testosterone, measured by equilibrium dialysis when possible rather than calculated
  • Sex hormone-binding globulin (SHBG), which determines how much of your total testosterone is actually bioavailable
  • Estradiol (sensitive assay), since testosterone converts to estrogen and both matter
  • LH and FSH, to distinguish primary testicular issues from pituitary or hypothalamic ones
  • Prolactin, particularly if libido is the dominant complaint
  • Complete blood count, comprehensive metabolic panel, lipid panel, HbA1c, and a full thyroid panel
  • PSA for men over 40 or with family history
  • Vitamin D, ferritin, and B12, which frequently masquerade as low-T symptoms when deficient

SHBG is the number most often overlooked. A man with a total testosterone of 550 ng/dL and high SHBG can feel just as symptomatic as a man with a total of 300, because his free (usable) fraction is squeezed. Insulin resistance tends to lower SHBG; aging, alcohol, and hyperthyroidism tend to raise it.

Making Sense of the Numbers

The standard reference range for total testosterone in most U.S. labs runs roughly 264 to 916 ng/dL, but that range was built from a broad population that includes many men who are themselves symptomatic. Falling in the bottom quartile of "normal" while feeling terrible is not the same as being optimized.

We interpret labs in the context of the whole picture: symptoms, sleep, body composition, training history, alcohol intake, stress load, and other lab findings. A man with a total testosterone of 380, low free T, elevated estradiol, and a BMI of 32 has a very different situation than a lean, well-slept man with the same total testosterone and normal free T. The first often responds beautifully to weight management, better sleep, and addressing insulin resistance before any hormone intervention is considered. The second may be a candidate for a different conversation.

What Comes Before Prescriptions

Any responsible hormone optimization plan starts with the lifestyle inputs that move testosterone naturally. These aren't token gestures. In men with mild to moderate deficiency, they can shift total testosterone by 100 to 200 ng/dL over three to six months.

  • Sleep: Fewer than six hours per night reliably suppresses morning testosterone. Sleep apnea, which we screen for aggressively, is a common hidden driver.
  • Body composition: Visceral fat is metabolically active and converts testosterone to estradiol via aromatase. Losing 10 to 15 pounds of fat mass often meaningfully improves labs.
  • Strength training: Compound lifts, done consistently, support both testosterone production and androgen receptor sensitivity.
  • Alcohol: Regular heavy drinking suppresses the HPG axis. This is one of the most reversible variables we see.
  • Micronutrients: Vitamin D, zinc, and magnesium deficiencies each independently affect testosterone production.

If, after addressing these, symptoms and labs still warrant it, we discuss hormone optimization protocols in detail: what they involve, monitoring requirements, fertility considerations (this matters even for men who think they're done having children), and realistic expectations. Questions to Ask Whoever Evaluates You

The quality of your evaluation depends heavily on who's doing it. Reasonable questions to ask:

  • Are you drawing my labs in the morning, and will you repeat them to confirm?
  • Are you measuring free testosterone directly or calculating it?
  • How will you factor SHBG and estradiol into interpretation?
  • What will you rule out before recommending hormone therapy?
  • If I start a protocol, what's your monitoring schedule and what are the exit criteria?
  • How do you handle fertility preservation?

If the answers are vague or the plan is "come in, get a shot, see you in three months," find someone else.

The Longer View

Testosterone evaluation is one piece of men's health, not the whole thing. Cardiovascular risk, metabolic health, cancer screening, mental health, and musculoskeletal maintenance all deserve attention alongside hormones. Men who focus exclusively on their T levels while ignoring their blood pressure, ApoB, or unaddressed sleep apnea are optimizing the wrong variable.

Done well, though, understanding your hormonal status gives you real information about how your body is running and where the leverage points are. It's a data-rich conversation, and it deserves a physician who will actually sit with the data.
